NHS continuing healthcare (CHC) is a package of care funded solely by the NHS for individuals who have significant ongoing healthcare needs or have a rapidly deteriorating condition that maybe entering a terminal phase, (fast track).

The GP practices in Lewes are offering ‘On Track’ Live Well Feel Better group sessions for people who have already had one-to-one health coaching and feel they would benefit from this continuation of support. The upcoming groups are as follows: Every other Wednesday at 6.00-7.30 pm

Upcoming groups are held on Wednesday evenings from 6.00 to 7.30 pm:
June 6th, June 20th, July 4th, July 18th, Aug 1st, Aug 15th, Aug 29th, Sept 12th, Sept 26th.
